Our Clinic is Closing
It has truly been a privilege to care for the people of this community for the past eleven years, and we thank you for trusting us with your healthcare needs.
We are no longer seeing patients at Crown Clinics, but will be happy to assist you in transitioning care to another provider. However, we cannot legally send your records to another entity without your written authorization. Please download and complete the “Release of Information” form. Either mail the form to 337 11th St. SW Spencer, IA 51301, or scan it and email it to this address: contact@crownclinicsmobile.com. Any requests sent by Oct. 31, 2024 will be honored. Be aware that we have thousands of records to transfer, and are working at this in the order that we receive the requests.
Medication requests need to be sent to your pharmacy. If you’ve had a recent evaluation and your lab work is current, medications may be refilled through the month of August. Any other questions can be emailed to contact@crownclinicsmobile.com.
